Working with partner agencies

Connexions Northumberland is currently working with a range of young people, families and partners agencies on a number of developmental issues, with the aim of improving services to young people and their families.

Some of these developmental issues include;

  • Multi-agency alignment of services within the developing Northumberland Families and Children’s Trust - Locality and Inclusion Integrated Teams and the emerging Integrated Youth Support Service.
  • Early identification and tracking of young people with complex needs - with the aim of sharing information between services to support the transition process, early identification of likely budgets and to support commissioning of bespoke provision and services.
  • Person Centred Planning and Reviews Pilots – to work with young people and families to obtain their views and promote a person centred, holistic approach to the transition process.
  • Developing a multi-agency good practice health check for the transition process.
  • Working with colleagues from the North East regional LSC and Connexions Services to develop additional publications, including differentiated materials for young people with special educational needs.
